Questions: A 23-year-old male presented to the casualty with high fever. He was tested and found to be positive for HIV four years prior to admission and his general state of health was good prior to admission. The patient was brought to emergency after being found confused, shivering and unable to respond appropriately. He had complained of a headache for three days prior to admission. On examination the patient was delirious and appeared toxic. His temperature was 39.8°C, pulse 108 beats/min, and respirations 48/min. Blood pressure was 156/98 mmHg. The patient was unable to respond to verbal commands. He appeared irritable. Suggest the DDs in this case. Write briefly about the investigations and treatment.
