The fourth ventricle is located within the brainstem, between the pons and cerebellum. Its floor features several nerve nuclei and white matter tracts. The median sulcus divides it into symmetric left and right halves.

Key structures found on the floor include cranial nerve nuclei for CN V-XII (trigeminal to hypoglossal), as well as the vestibular area, cochlear area, and the central canal. The fourth ventricle is filled with cerebrospinal fluid and continuous with the cerebral aqueduct above it.
